Nurse Case Manager/HTC Nurse

As a Nurse Case Manager for St. Jude Children’s Research Hospital, you’ll have the opportunity to serve the patients and families in our Bleeding Disorder and Thrombosis Clinic/Hemophilia Treatment Center (HTC). Alongside a tight-knit team of experts -- advanced practice providers, social worker, academic coordinator, pharmacist and physician -- you will be the nucleus of care and support for these vulnerable children, navigating the first touch for families. Your role is to serve as a coordinator and primary contact for our patients’ care, be an educator, and a liaison and champion during their diagnostic evaluation, treatment, and routine health care follow up. Your role will be multi-faceted and include being a content-expert for patients and families while simultaneously managing many relationships with external partners including primary care physicians and other treatment partners, home health service companies, specialty pharmacies, and community organizations. Together with the HTC Director, you will oversee the clinical and research activities as the HTC coordinator and participate in the support of local, regional or national quality improvement or research projects.

Requirements

  • Graduation from an accredited RN program required
  • Bachelor's degree in Nursing (BSN) preferred
  • Two (2) years nursing experience required
  • Research, case management and/or education experience preferred
  • Must possess a current Tennessee State Board of Nursing license if primary residence is Tennessee or a Nurse Licensure non-Compact state -OR- Must possess a current State Board of Nursing license in the state of primary residence if the state is a Nurse Licensure Compact state
