Full-time Hemophilia RN Case Manager for the Hemophilia and Thrombosis Treatment Center within the Hematology Oncology Outpatient Department. The schedule is Monday to Friday 0830-1700. Preferred Hemophilia and case management experience.
JOB SUMMARY:
The Case Manager for the Hemophilia and Thrombosis Treatment Center works with a multidisciplinary team to coordinate diagnosis and treatment for children, adolescents, and young adults with bleeding and clotting disorders.
Case Manager describes a variety of specialized nursing positions that provide assistance in planning, coordinating, and delivering patient care for selected patient populations across the continuum of care using the nursing process, ensuring compliance with physician orders and established policies and procedures. Case Managers provide coordination and care of patients requiring assistance and support in accessing and managing their health care, in partnership with the patient and family. They collaborate with other healthcare providers to ensure continuity of care and coordination of a wide variety of patient services. They use performance improvement methodology to design data collection systems/processes and identify opportunities to improve care/service for patients. Responsibilities include coordinating patient contact, patient education, and preparing patients/families for procedures/tests in collaboration with the health care team; collaborating with others to ensure continuity of care and coordination of a wide variety of patient services; identifying opportunities to improve patient care services; assisting clinic management to identify and develop operational needs for the clinic and to improve patient care services; and direct patient care.
MINIMUM QUALIFICATIONS:
Bachelor's Degree 3 years of experience California RN License BLS for Healthcare Providers, issued by AHA Must qualify for CCS Paneling Must have good problem-solving skills Excellent oral and written communication skills Able to manage large call volumes and triage patient care needs appropriately Experience working with children and adolescents with hematology and/or oncology diagnoses
PREFERRED QUALIFICATIONS:
Master's Degree 5 years of experience Clinical experience with children with bleeding disorders Bilingual - English/Spanish
The current salary range for this position is $49.94 to $85.94
