Position Summary
The Advanced Practice Provider (APP) will be an integral member of the multidisciplinary team at the Hemostasis and Thrombosis Center, providing specialized care to adult patients with congenital bleeding disorders and acquired factor deficiencies.
Working in close collaboration with the adult hematologist, the APP will deliver comprehensive, evidence-based clinical care in both outpatient and consultative inpatient settings.
What You Will Do
Provide direct patient care, including assessment, diagnosis, and management
Place and manage orders for labs, imaging, procedures, and medications
Participate in comprehensive care visits, including outreach and offsite clinics
Follow up on laboratory results and implement care plans
Round and consult with inpatient teams when HTC patients are hospitalized
Lead and assist in surgical planning, including preoperative, perioperative, postoperative, discharge, and home therapy management
Provide education and training to healthcare professionals and consumers, including presentations and written materials
Participate in HTC community events and professional training programs
Comply with and participate in research activities, including HTC and industry studies
Key Responsibilities
Perform comprehensive assessments, including history-taking, physical examinations, and interpretation of diagnostic results
Develop and implement individualized care plans in collaboration with the adult hematologist and multidisciplinary team
Manage treatment regimens, including factor replacement therapy, rebalancing agents, prophylactic protocols, and gene therapy
Coordinate and participate in multidisciplinary comprehensive care visits
Serve as the first point of contact for questions regarding orders, scheduling, and laboratory work from infusion RNs, pharmacies, and administrative staff
Place and update orders in EPIC in a timely manner
Manage the team inbox in EPIC, including refills, patient advice requests, abnormal lab values, pending order signatures, prior authorizations, and peer-to-peer medication approval requests
Provide patient and caregiver education on disease processes, treatment regimens, and preventive care
Collaborate with physicians, nurses, social workers, pharmacists, genetic counselors, and physical therapists to ensure seamless care
Contribute to quality improvement initiatives and clinical research activities
Maintain accurate and timely documentation in EPIC
Engage in continuing education to maintain credentials and stay current with clinical advances
Working Conditions
Primarily outpatient clinic-based with inpatient consultation as needed
Participation in outreach and potential offsite clinics
Standard daytime clinic hours with flexibility for procedural planning or urgent needs
Participation in community education and outreach activities related to the HTC
Education Qualifications
Master of Science in Nursing (MSN), or
Master’s degree from an accredited Physician Assistant educational program
Experience Qualifications
One or more years of experience as a Nurse Practitioner or Physician Assistant preferred
Skills and Abilities
Demonstrated leadership skills
Strong interpersonal skills
Proficiency in accessing patient medical data from the UHHS mainframe
Working knowledge of Microsoft Office and Outlook
Licenses and Certifications
Current Registered Nurse (RN) license in the State of Ohio and Nurse Practitioner (NP) license in the State of Ohio, or
Current Physician Assistant (PA) license in the State of Ohio
Basic Life Support (BLS) required
Advanced Cardiac Life Support (ACLS) preferred
